Stanford Heart Network (SHN) - Our Privacy Policy

Because your privacy is very important to us, the Stanford Heart Network (SHN) operates by these principles:

The SHN website (Site) is maintained by HealthCalc.Net, Inc., 5068 W. Plano Parkway, Suite 290 Plano, TX 75093 (HCN), a Division of Health Fitness Corporation (HFC) . Project staff at Stanford Medical School are responsible for the content of this Site. We can be reached by e-mail at webmaster@healthcalc.net.

HCN uses Secure Socket Layer (SSL) technology to encrypt all SHN responses so that prying eyes cannot see the answers given by any individual. Our goal is to maintain maximum security of your information while at the same time provide useful services to you, your personal physician or other health care providers who are assisting you with your heart attack and stroke prevention program. The only people other than yourself who will have access to your personal data are those who provide care or services to you. These persons all know the importance of keeping your data confidential.

We explicitly ask for your permission when we need information that personally identifies you (Personal Information). Personal Information is needed to evaluate your risk for heart attack and stroke and to provide you individualized information based on your own data. If you give approval, we may occasionally inform you by e-mail of new SHN features and services based on your personal data. In all cases, we will explicitly inform you as to the requirement and/or use of requested Personal Information. Neither SHN nor HCN will release any of your data to anyone not directly involved in the conduct of this project without your written permission.

Data you provide to SHN may be used anonymously to produce group reports that can be used to study population health characteristics and benefits resulting from various heart disease prevention programs. For example, we may compare the number of persons with high cholesterol at one clinical site versus another site or the change in cholesterol levels in one group versus another group.

We may place a small text file called a "cookie" in the browser files of your computer. The cookie itself does not contain Personal Information. The main thing it does is help us keep track of the name of the account and who you are as you use various features of the SHN Site. By doing this, it helps us keep your data secure. It enables SHN staff to know when you have accessed the Site and what sections of the Site you used. This helps us keep track of how much and for what reasons the Site is being used.

This Site will contain no advertisements and information about SHN participants, including e-mail addresses, will not be provided to any other organization.

This site gives users the option for removing their identifying information from the SHN database, to not receive future communications or to no longer receive our service. Visitors should login with username and password. Once login is complete, go to "Settings" and look for the link to Delete user information.

This site gives users the option for changing and modifying information previously provided. Visitors should login with username and password. Once login is complete, look for the link to Settings or Edit Responses.

This site gives users the option for removing their identifying information from the SHN database, to not receive future communications or to no longer receive our service. Visitors should login with username and password. Once login is complete, go to "Settings" and look for the link to Delete user information.

If at any time you believe the SHN Site has not adhered to these principles, please notify us by email to webmaster@healthcalc.net, and we will use all reasonable efforts to promptly determine and correct the problem.
