About the Stanford Health Network

The Stanford Heart Network (SHN) was established in 1999 to assist health professionals from around the world become more effective in preventing heart attacks and strokes. Its major purpose is to be a resource for doctors, nurses and other health professionals who provide heart disease prevention, outpatient treatment and rehabilitation services to specific populations of patients.

The Network supports its Members in the development and implementation of multifactor risk reduction programs for high-risk patients. Computerized cardiovascular disease risk screening questionnaires and forms to assess clinical status and quality of life are available for use by Members and data are collected to assist participant and program evaluation. To assist in the training of staff and program implementation, the internet site includes patient management guidelines, prevention/treatment algorithms, educational materials, "health trackers" to help with risk factor management, clinical measures to assist participants and health care providers to set goals and track progress and links to selected sites.
